Is there a way to extract a list of all Entities in a diagram along with there descriptions?

For internal or external purposes? I think the easiest approach here is to export the diagram to the XML format. Then you’ll end up with a simple text file (ASCII) which you can load in any random text editor and it’ll have all the elements and their properties laid out in an easily processed format.

You can create by Doc. Composer > Build Doc from Stratch, select the diagram on Diagram Navigator, then drag&drop template to the document.
If you want the name and description of all entities only, you can drag Loop template with this setting:
